Please Help! What do i do

I have a credit report that only has five accounts that have all gone into collections.
As for SOL one ends 10/2011 do i leave this one alone or dispute it?
others are pretty recent.

can anyone please tell me EXACTLY what to write in the first dispute letter?

also, there are quite a few inquiries. do i dispute the inquiries as well or wait to do these later?

i appreciate your help
oh one last thing, do i send a copy of drivers license as well?
