Doctor Withdrawal Agreement Letter

Ok I spoke with the doctor office today they may be willing to withdraw collections request from CA. I explained to them that the CA will not respond and I provided them with the clause in there contract that allows them to do so. Does any one have a agreement letter that would work in this situtaion?
