I don't think there is a sample letter for that....
You just need to be sure and let them know that they are reporting an amount due that YOU were never responsible for. So they need to delete IMMEDIATELY since it is inaccurate and damaging your report/scores. Depending on how aggressive you want to be you can even tell them you will be seeking legal counsel if you do not response within xx number of days (depending on how quickly you want them to respond - 14 would be fine).
And yes, send the letter that you have as proof that it wasn't your responsibility to pay that and that it was a mistake.
Then keep after them. I would call to make sure they receive the fax or letter(get the name of the person that tells you they rec'd it)..........and then keep checking back with them. Be polite but be firm (and hopefully annoying enough for them to want to do this quickly).
If you do send it via snail mail.......be sure to send it CMRR.
